Affidavit of Address Form
If you are a NEW Shareholder of Cape Fox Corporation, you MUST fill out an Affidavit of Address form.

Affidavit of Name Change Form
If your name changes due to marriage or any other legal action, you must complete an Affidavit of Name Change form.
Please fill out the affidavit using your current name. Provide your former name under number one (1) of the affidavit. Provide the legal action and your current name under number two (2) of the affidavit. The affidavit MUST be signed in the presence of a Notary Public and MUST include a copy of the legal decree.

Alaska Statutes Excerpts from the Alaska Uniform Transfers to Minor Act
If a Cape Fox Corporation Shareholder is under the age of 18, a custodian must be appointed (usually the parent or legal guardian) until the Shareholder turns 18 years of age.

Appointment of Custodian Form (and Custodian's Consent to Appointment)
If a Cape Fox Corporation Shareholder is less than 18 years of age, a custodian MUST be appointed. The shares that the Shareholder are entilted to recieve shall be held by a custodian (usually the parent or legal guardian) until the Shareholder turns 18 years of age.
A person making a transfer of stock, whether by gift, devise, or other method, may nominate a custodian. In the absence of a nomination, the custodian shall be determined under the order of priority set out in the Alaska Statutes Excerpts from the Alaska Uniform Transfers to Minor Act provided above. The appointment becomes effective upon the corporation's receipt of the custodian's written consent to the appointment.

Recipient Shareholder ID Card Form
New Shareholders must fill out a Shareholder ID Card Form to request a Cape Fox Corporation Shareholder ID. A recent profile photo should be included with the form. If you do not have access to a profile picture, you can come to the Cape Fox Corporation Saxman Office to get a photo taken.

Recipient Form (Accepting Shares)
Each recipient receiving a gift of shares MUST complete and have notarized a Recipient Form. If the recipient is under 18, the custodian of the recipient (see Appointment of Custodian Form above) MUST complete and sign the form on behalf of the minor.
On the back of the form is an optional Release for Publication form. The recipient or custodian may sign this form granting the release of the recipient's name in the Totem Times and other Cape Fox Corporation publications.

Shareholder Form (Gifting Shares)
A Shareholder who wishes to make a gift of shares MUST complete and have notarized a Shareholder Form. Cape Fox Corporation does not accept Shareholder Forms by friends or relatives of the Shareholder.

Shareholder Forms submitted using the paper form below must include the following: full name of the donor as it appears within the Cape Fox Corporation Shareholder records, date of birth, social security number, complete mailing address, and a valid signature from the Shareholder signed in front of a Notary Public.
On the back of the form, please complete the following: the full name(s) of the recipients of the gift of shares, relationship to donor, recipient's complete mailing address, date of birth, social security number, and numberof shares to be gifted.

Testamentary Disposition Form
Each Shareholder MUST complete and have notarized a Testamentary Dispostion Form which will allow Cape Fox Corporation to follow your wishes in regard to who will receive your shares upon death.
Shares in Cape Fox Corporation may be transferred by testamentary disposition using the Cape Fox Corporation Testamentary Disposition Form, a Last Will and Testament or a Holographic Will. Shares transferred by testementary disposition to a Native or descendant of a Native shall remain Class A Voting stock of Cape Fox Corporation. Shares transferred to a person who is not a descendant but is an Alaska Native as defined by ANSCA to mean a citizen of the United States who is a person of one-fourth or more Alaska Indian (including Tsimshian Indians not enrolled in Metlakatla Indian Community), Eskimo or Aleut bood, or combination thereof. The term includes any Native as so defined either or both of whose adoptive parents are not Native. Shares transferred to a non-Native shall become Class B non-voting stock of Cape Fox Corporation unless and until such shares are transferred back to an eligible Native or Native descendant Shareholder.

Updated Shareholder ID Card Form

Every new shareholder must fill out a W-9 form. The recipient should include his of her name, address, social security number, and signature. The remaining boxes are used for businesses and can be left blank in this case.

Principal Life is Cape Fox Corporation's current Life Insurance Provider. Principal Life helps protect our Shareholders and their families by giving the assurance of knowing their loved ones will be provided for. Shareholders' loved ones can use the benefit to cover final expenses, everyday living expenses, and long-term obligations that may otherwise cause financial strain. To ensure your loved ones are provided for, please complete the Beneficiary Designation form.

Information Request Form
Click on the "View form" link below to access the Information Request Form. The Information Request Form can be used to request to review various business records for a limited purpose. The Information Request Form must state the information requested and a detailed purpose of request.
All information provided by Cape Fox Corporation is confidential; and shareholders must agree to protect all information from unauthorized use and dissemination - including to other shareholder without the approval of Cape Fox Corporation.
